Please be sure to check if your plan covers acupuncture and/or massage. If you do not see your insurance listed, please email us. All copay and/or coinsurance amounts are due at the time of service. Patients are always responsible for all deductible amounts. We do NOT currently accept credit cards.
If you are 18 or over, you are legally responsible for your own charges, regardless of who you live with, who claims you as a tax deduction or who is the insurance plan subscriber (such as a parent or other designated individual who provides you with insurance). If the patient is under 18, parents OR the legal guardians are responsible for payment.
You are fully responsible for knowing the specifics of your particular insurance coverage. Examples of these specifics include: co-pays, deductibles, second opinions, pre-authorizations, preferred providers, referrals, covered and non-covered services and preferred hospitals and facilities.
Our office may be required to request pre-authorization from your insurance company for hospital and surgical visits or specialist referrals. Please note that this does not guarantee payment. Payment is made according to your contract. The clinic cannot accept responsibility for collecting your reimbursement or negotiating a settlement on a disputed claim. We can provide you with the necessary medical information to assist you.
UNINSURED PATIENT DISCOUNT PROGRAM
THE ART OF MEDICINE PLLC is committed to serving the healthcare needs of its patients and has established the Uninsured Patient Discount Program to assist both uninsured and under-insured patients.
THE ART OF MEDICINE PLLC considers a patient: A. uninsured when a patient has no health insurance AND B. under-insured when a patient’s primary, secondary and other health insurance will not cover a specific service or procedure at our healthcare facility. Insured patients must follow the rules of their insurance plan.
Uninsured and under-insured patients will be charged a discounted rate based on current Federal program rates as established by the Centers for Medicare Services. If there is no Federal coverage for a specific service, THE ART OF MEDICINE PLLC will offer the patient a flat discounted rate. This rate will be similar to the discount based on the Federal program rates for other services.
All fees must be paid in full at the time of service to receive the uninsured/under-insured patient discount. Failure to pay in full at the time of service will result in a bill for the full amount specified in our standard fee schedule which may be more than double the Uninsured Patient Discount Program rate. Estimated fees do not necessarily represent our standard fee schedule, but rather may
reflect discounts based on Federal program rates. Estimated fees are subject to change.

Follow-up Visits for with a Medical Doctor for Medical Acupuncture and Osteopathic Manipulation $150
55 minute Medical Massage performed by a licensed Massage Practitioner $55
