Once completed, please fax it to the number on the form, mail it to the address on the form, or drop it off at the clinic. The completed form is required before an appointment can be scheduled.
After receipt, our office will contact you via email and/or phone to schedule an initial consultation. Please be aware that we are sometimes booked out several weeks in advance, so please plan accordingly.
